I've tried copying the file and pasting into the same directory with a new name and also moving it to other directories too but it's still not being picked up by the media scanning.
(NOTE: I'm still on version 0.72)
If I run ProcMon on it I can see the directory being processed and all the .jpg/.avi files in the folder being processed but the .mkv is just being ignored totally no matter where I put it or what it's called.
There's a section on procmon that reads:
Here you can see that the .mkv file exists in the directory and also the Test File.mkv which is the renamed version.
Below is the full extract from ProcMon although I'm not sure how clear it is. It's easier to see if you copy/paste to notepad.

Yes, it's picking up all of my other .mkv files that are all in the same kind of folder structure.
i.e. I have Dorian Gray in the folder
Videos\DramaHD\Dorian Gray(2009)
It is the only .mkv file in there and has the same kind of files such as mymovies.xml, folder.jpg in that directory too.
The file is about 1.5Gb smaller though so not sure whether size might be a factor although I'm sure I have larger files that are being listed too.
I've checked the properties on the files too and they all seem the same so can't really figure out what the difference is.
I've also run this script to see whether the .mkv file would get listed and it does although I'm not sure whether that's particularly relevant or not:
