Update: in another test FFMPEG did not terminate even after closing the client connection, so it may not be terminating reliably. It is also strange that it doesn't try to terminate until the client connection closes, rather than when playback stops.

Ok, I changed the setting to > 720 and it is working correctly now. Is this information stored in the device profile? If so, you may want to update your iPad profile accordingly.

In my case, the m4v files are H.264 encoded, so is 720p
640 looks correct for all other formats though (per iPad video spec):

H.264 video up to 720p, 30 frames per second, Main Profile level 3.1 with AAC-LC audio up to 160 Kbps per channel, 48kHz, stereo audio in .m4v, .mp4, and .mov file formats; MPEG-4 video, up to 2.5 Mbps, 640 by 480 pixels, 30 frames per second, Simple Profile with AAC-LC audio up to 160 Kbps, 48kHz, stereo audio in .m4v, .mp4, and .mov file formats; Motion JPEG (M-JPEG) up to 35 Mbps, 1280 by 720 pixels, 30 frames per second, audio in ulaw, PCM stereo audio in .avi file format
I think this is a specification (720p) for files that are stored locally.
